They do this in order to justify and make real ("validate") their belief which they know others will not believe. It is a passionately interesting game to them - and a very dangerous game to their victims. I witnessed this first hand. My exposure ended in 2006 - 15 years ago, so I have had time to read and analyse. There have been lots of famous narcissists in history, of course. They were mistakenly believed by the crowds and became leaders - obvious ones like Adolph Hitler and Donald Trump. We laughed at Donald Trump but he never betrayed any embarassment nor did he apologize for any of the obvious lies he told. These are very telling omissions. He just didn't register the "discontinuity" in his behaviour which we did. And I can make a good case that Boris Johnson is a narcissist (see his previous employer Max Hastings: https://www.dailymail.co.uk/debate/article-2215342/If-Boris-Johnson-Prime-Minister-Im-plane-Britain-says-Max-Hastings.html). Of course understanding how a narcissist is manipulating others is hard work. Remaining detached emotionally is the province of psychiatrists and psychologists who are trained to do this. It is not doing a deluded patient any favours to go along with his delusion. Dr Paul McHugh was the last independently minded Chair of the Dept of Psychiatry at the Johns Hopkins University Hospital which pioneered and then stopped sex surgery in 1979. Dr McHugh argued to retain that ban, but they restarted in 2017 as a result of WPATH, an organisation set up and run by GD sufferers - with no independent oversight.
