Thanks Hans,

Yes, in the links you have given, LTN1-20 is "Local Transport Note 1 20" and this is used by highway engineers to actually design roads and has been substantially updated with requirements for cycling.  It is similar to the London Cycle Design Standards but applies to all of England.

The last link is to proposed changes to the Highway Code which I haven't looked at.  I believe the general approach is to emphasise that road users who can do the greatest harm have the most responsibility.
