What about paying for GP or hospital appointment and reclaiming a portion later?   The repayments could be based on your income status, the richer pay a bit more the genuinely impoverished get the full amount back, or are exempt to start with.   I think the French do it that way but I wonder if it would stop a lot of "time-wasters" bunging up the system?   Who knows, it's just a thought.
