You can't judge from my photos the extent of any hazard from the state of the pavement.  It's obvious that the pink top layer has worn away at the edges, but it's a thin layer.  There are far worse trip hazards elsewhere in Chiswick.  As Sam Hearn says, pavement defects should be reported via Hounslow's website, but with full address details and based on actual experience, not making assumptions from a photo.
