For disruption the A4 is worse  - for instance, without it there would be no need for subways in Chiswick.

As far as noise is concerned, you're right - the A4 doesn't have much effect beyond the first row of houses facing the road (see the London noise map at http://81.187.187.47/). But only those south of Chiswick Station have to put up with plane noise* even approaching the levels for houses by the A4. I'd guess the numbers affected are roughly comparable, though the noise of living along the A4 is undoubtedly worse.  

More people in Chiswick can hear the planes rather than the A4, but it's at a rather low level and will be drowned out by any local traffic. Overall, silent cars rather than silent planes would do more to improve the quality of life in Chiswick, even with a third runway.
