Joking aside, in theory there's no real reason why you shouldn't be able to now, you could have a miniature camera on the watch, Bluetooth connection to the phone and a 4G signal which should be more than enough bandwidth. I would say that the limiting factor is probably precisely what puts a lot of people (certainly me) off smart watches. I don't want to have to plug a watch in to charge every night, and if you were doing something power intensive like video calling, you'd need to carry a booster charger around with you. Not terribly Thunderbirds I'm afraid.
