Whether the vehicle made contact, or whether a pothole played any role in the cyclist losing balance, the fault here rests entirely with the driver for pulling in too soon, which is indeed beyond dispute.

Either the driver after pulling out to pass paid no further attention to the cyclist when they pulled in and drove on unaware of the consequences of their manoeuvre, or they were aware that the cyclist had gone down and drove off to escape the consequences. Neither possibility changes the culpability for their driving, and indeed there is a case to be made that the penalty should really be the same whether or not there was any injury to the cyclist. Of course if the driver was aware that the cyclists had gone down, that is an additional offence.

I think injury or death is used to assess how dangerous driving had been based on the consequences, given often there is little actual evidence of the driving itself. In this case we have the evidence, and that driving would be no less dangerous if the cyclist had been able to avoid injury.
