I'd say James' comment summarises the problem exactly. Although road design could be better (number one for me would be eliminating slip roads from everywhere but motorways) the problem is the attitude of motorists. Which is that the road is built for motor vehicles so get off it.

In Holland there was a huge swell of public opinion blaming motorists for deaths of children on the roads, redesigning roads to prioritise non-motorised forms of transport was therefore leaning against an open door.

In the UK we have the opposite attitude. Motorists get away with murder on the roads by simply saying 'I'm sorry I didn't see them' - there isn't a jury that wouldn't let them off and so most of the time motorists that kill aren't even prosecuted - despite continual attempts by the gov to tighten things up. And frankly apart from a few nobody seems to care - it's a mystery to me.

After years of cycling I don't see any immediate prospect for improvement - you can't segregate all roads and until you do you can't stop people being scared off the roads by the careless or deliberate actions of motorists. I put my faith in self driving cars as the eventual solution in the UK. Dutch, German and Danish road layouts don't work with the attitudes of UK drivers.
